Initial Examinations are done on a first-come, first-served basis. No appointment is necessary. This also applies to applicants referred back by the Embassy of Canada for further tests

The following applicants should contact SLEC at 521-0020 for their examination/testing schedule:

  • Applicants referred by other screening facilities
  • Applicants for DNA testing

Applicants who have been scheduled for additional examinations (e.g. Sputum collection) and/or DNA Testing must come on their designated examination date and time.

REQUIRED DOCUMENTS

  1. For Regular, Immigrant, Working, Student, Visitor Categories
    1. Passport/ national ID (SSS, postal ID, voter's ID, PRC ID. driver's license, school ID) and birth certificate
    2. Two (2) PHOTOCOPIES of passport/national ID and birth certificate (see format)
    3. 6 pieces recent passport photos (white or blue background)
    4. Medical Instruction from the Embassy (Embassy Form 1017)
    5. Photocopy of Embassy Form 1017
  2. For Sponsorship Immigrant Category
    1. Passport
    2. 6 pieces recent passport photos
    3. Appendix C or D Medical Instructions form

Whenever applicable, please bring the following:

  • Old chest x-ray films not less than 3 months prior to the medical examination
  • Detailed medical certificate regarding a previously-treated or currently managed clinically significant illness which should contain the following information:
    • duration and date of treatment
    • surgical procedure or medical intervention done
    • complete final diagnosis
    • outcome of treatment or treatment plans and prognosis, if available

15 y/o and above Physical Examination, urinalysis, CXR, HIV, Syphilis

Rates effective August 2, 2012.

Specialty consultations available within the clinic are performed free of charge. Referrals from other clinics will be charged accordingly.

St. Luke's Medical Center Extension Clinic does not accept cheque or credit card payment. Fees must be paid in Philippine Pesos. Other examinations, when required, will be charged accordingly.

REMINDERS:

  1. Applicants are advised to take their meals prior to coming over for their medical examination since food is not allowed within the premises and all the initial blood and urine tests do not require a fasting state. There are restaurants near St. Luke's Medical Center Extension Clinic and drinking fountains are available within the building.
  2. Females within the reproductive age group are advised not to schedule their medical examination during their menstrual period. Otherwise, they will be required to come back one week after the last day of their menstruation.

  • Medical results and documents will be released not on a first-come first-served basis but depending on the physical examination, x-ray and laboratory results, which will be conducted and processed in accordance with the guidelines set by the Canadian Embassy - Health Management Branch.
  • The medical report, and chest x-ray are directly submitted by St. Luke's Medical Center Extension Clinic to the Canadian Embassy in Makati. For offshore applications, medical reports and x-ray films are submitted via courier.

  • The medical report, passport and visa photographs are submitted by St. Luke's Medical Center Extension Clinic directly to the U.S. Embassy, with few exceptions. In most cases, only the cd of Chest X-ray and Vaccination Documentation Worksheet are given to the applicants in the releasing section.
